Bebashi Transition to Hopeis seeking a dedicated and experienced Community Health Worker Supervisor to join our team. This role will report to the Chief of Strategic Initiatives.
Position Summary:
This position is responsible for the supervision of a small team of three or more community health workers (CHWs) who are working to improve health and well-being by addressing racial ethnic and economic disparities in health status.
CHWs are conducting community outreach activities and working directly with individuals and families with low-incomes unmet needs and/or who are eligible for Medicaid benefits in Philadelphia PA.
Community outreach activities aim to support adoption of healthy behaviors by assessing community health needs and providing information on available resources social supports and navigation of resources.
The CHW Supervisor will lead the team to address the impact of social determinants of health and achieve health equity and systems-level change(s) through community collaborations to mobilize a community response and engage diverse partners including community residents.
The Community Health Worker Supervisor provides leadership oversight and support to Community Health Workers (CHWs) delivering community-based services.
This role ensures high-quality service delivery compliance with program and funder requirements accurate documentation and effective coordination between CHWs clinical partners and internal departments.
Key Responsibilities:
- Plan and coordinate outreach supervise individual and family assessments assess training needs provide health education home visiting and service coordination performed by CHWs.
- Develop in conjunction with the coordinating agency an overall workplan including outreach and service strategies for the in the targeted geographic area.
- As a member of an integrated care team works closely with other providers across the organization and partners to ensure progress and implementation of the workplan including leading structured meetings and identifying/resolving issues.
- Prepare and present reports and evaluations defining project progress problems and solutions.
- Establish relationships promote collaboration and coordination with other community health and human service providers and diverse community partners.
- Conduct community and public education sessions to promote initiatives or programs including primary prevention education; chronic conditions/disease management; screening and testing.
- Participate and/or lead a community action board engaging community members through community and civic engagement activities.
- Provide or ensure clients have access to education classes and enhanced social supports (e.g. affordable and stable housing; food insecurity or utility services threat).
- Ensure that clients are engaged with providers to support access to systems of healthcare to support primary and specialty care social services public benefits and other community resources.
- Conduct case conferencing with CHWs to monitor client services referrals and appropriate follow up.
- Design and implement client surveys to evaluate client satisfaction with CHW services.
- Coordinate with agency staff to develop documents (educational materials forms signage etc.) and procedures that support a health literate environment and enhance client understanding of health information.
- Prepare required reports and other written material regarding program implementation and activities.
1. Billing Compliance & Documentation Oversight
- Ensure CHW services meet documentation standards required for billing and reimbursement
- Review case notes service logs and encounter documentation for accuracy timeliness and completeness
- Verify documentation supports billed activities and service codes
- Monitor compliance with service authorization limits and eligibility criteria
- Support correction of documentation errors and prevent billing denials
- Maintain audit-ready files and assist with internal and external audits
2. Regulatory & Grant Compliance
- Ensure services comply with Medicaid/managed care/payer and grant requirements
- Monitor adherence to scope of work and performance metrics
- Support reporting requirements including service units outcomes and demographic tracking
- Maintain compliance with confidentiality laws (HIPAA) and organizational policies
3. Training & Quality Assurance
- Train CHWs on documentation standards billing rules and compliance procedures
- Develop tools and workflows to strengthen documentation quality
- Conduct regular documentation reviews and quality assurance checks
- Identify trends in denials errors or compliance risks and implement corrective actions
