How youll contribute
A Discharge Coordinatorwho excels in this role:
- Provides aftercare coordination for patients requiring ongoing medical or psychosocial support post-discharge.
- Creates and maintain updated community resource listings for aftercare services.
- Actively participates in the treatment planning process by scheduling aftercare appointments identified by the patients provider and therapist.
- Provides patients and families with resource information related to future care needs.
- Communicates proactively with clients families aftercare providers and referral sources.
- Drafts comprehensive discharge summaries and contracts for each assigned patient.
- Collaborates with the interdisciplinary team including physicians utilization review and nursing staff.
- Completes all required hospital and patient documentation thoroughly and in a timely manner.
- Applies appropriate crisis intervention and de-escalation skills during emergencies.
- Maintains confidentiality and ensure all HIPAA standards are upheld.
- Demonstrates working knowledge of clinical regulations including TJC Medicaid and state/federal laws.
What were looking for
Applicants should have a Bachelors degree in Social Work Counseling or equivalent (required). Additional requirements include:
Prior experience with psychiatric and chemical dependency patients preferred
CPR and De-escalation certification required or obtain with 30 days of hire.
Strong organizational communication and documentation skills required
