Were expanding and are looking for a dedicated Registered or Enrolled Nurse to join our Access Aged Care team onsite in Karrinyup.

What Youll Do
- Work alongside RACH care teams as the on site Access Aged Care nurse.
- Facilitate and Organise GP/Specialist rounds (both in-person and telehealth) to ensure smooth timely care.
- Prepare organise and triage referrals to maximise efficiency and outcomes.
- Conduct bedside assessments and clinical reviews.
- Act as the communication bridge between RACHs GPs and families.
- Deliver excellent customer service with empathy and professionalism at every interaction
- Build strong relationships with residents families and facility partners.
- Use your clinical knowledge and IT skills to ensure quality outcomes.

What Youll Bring
- Minimum of 2 years in an RN/EN role (post graduation) within Australia and AHPRA registration.
- Confident communicator with critical thinking skills
- Comfortable working autonomously
- Strong organisational skills and attention to detail
- Ability to build trust quickly with residents families and teams
- Keen to learn and confident in your clinical judgement
- Technologically proficient - Computer skills are a must!
