Job Responsibilities

Coordinates total patient care by conducting comprehensive health and psychosocial evaluation monitoring the clients condition promoting sound preventive practices coordinating services and educational and training activities.

Submits clinical notes no less often than weekly; submits progress notes and other clinical record forms outlining the services rendered as indicated.

28 Points Per Weekend
2.5 points for start of care
1 point per visit
1.5 Points for ROC and Recent OASIS Discharge
Assess the needs of the patient to identify specific nursing interventions.
Maintain home health equipment supplies and inventory.
Assist patients to develop health care plans.
Address patients physical psychological social and spiritual needs.
Address the effects of stress on patients families and coworkers.
Administer medication to patients in their homes.
Maintain staff attendance records.

Job Requirements

Registered Nurse in the State of Florida-Required Upon Hire
2 years of recent Home Health Experience-Preferred
Level 2 Background Check
