Job Summary

The Housing Programs Department serves households who are experiencing or at risk of homelessness in Hennepin County. Our programs include Prevention Diversion Coordinated Entry Assessments Fast Track Rapid Rehousing and Long-term Supportive Housing for households with children and adults without children. By offering programs of various intensity and length we hope to be able to provide the level of service each household needs to be successful. The goal in all of our housing programs is to end homelessness by assisting households in stabilizing their housing and achieving self-sufficiency.

The Housing Subsidy Coordinator is a key member of the Agate Supportive Housing Team with responsibility for administering the day-to-day operations of Agates Housing Trust Fund subsidy program as well as supporting the basic needs of households who are not enrolled in case management services. Housing Trust Fund duties include monthly financial processes inspection scheduling rental assistance calculation and eligibility paperwork completion. The remaining role responsibilities are to support a caseload of participants who do not qualify for Agate case management through providing in-office and phone-based paperwork support and personal needs supplies. This position works in close partnership with the Sr. Supportive Housing Manager and the Associate Director of Supportive Housing.

Position Responsibilities:

Program Coordination - Housing Trust Fund

  • Financial Management
    • Ensure accurate verification of participant and unit eligibility then perform calculations of allowable assistance.
    • Manage and track monthly rental payments and secondary check requests.
    • Coordinate with Contract Accountant to ensure monthly reconciliations and invoices are correct.
  • Paperwork & Data Compliance
    • Review participant files to ensure compliance with program requirements.
    • Monitor certification and termination dates for participants as required by program funders. Prepare and mail out relevant paperwork and notices to participants.
    • Work closely with Agate Housing and Services program staff to ensure documentation for all income or housing changes that would impact a participants supportive housing are completed.
    • Ensure case management files are up to date and accurate in internal ClientTrack system.
    • Coordinate with Agate Housing and Services program staff and Housing Quality Standard Inspectors to ensure housing inspections are scheduled and related paperwork are complete according to funder guidelines.
    • Coordinate with the HMIS Data Specialist to ensure participant information is current and in compliance with required quarterly and annual HMIS reports to MN Housing and Hennepin County.

Participant Support Housing Support

  • Direct Client Support
    • Serve as the primary Agate contact for a caseload of Housing Support households who receive their case management from an outside agency via programs like CADI or Elderly Waivers. Communicate with the outside case worker and Agate Housing Location as needed for moves program recertification and crisis management.
    • Maintain a minimum of once-monthly contact with caseload through primarily in-office phone and email-based methods.
    • Offer quality contact with participants to maintain trust assist in crisis prevention and assist in meeting individual needs.
    • Host weekly office hours for enrolled Housing Support households to pick up home goods hygiene supplies laundry money and bus cards; submit their utility bills; and receive support completing benefits paperwork.
    • Support and work with participants with diverse identities and experiences including young participants; participants with cultural backgrounds of historical/generational trauma; participants experiencing past or current domestic violence; participants struggling with mental physical developmental and chemical health challenges; and with persistent low income.
  • Paperwork & Data Compliance
    • Ensure case management files are up to date and accurate in internal ClientTrack system including accurate completion of documentation and case notes.
    • Ensure all clients on caseload are actively enrolled in the Housing Support program through supporting them with benefits paperwork.
  • Performs all other duties as assigned.

Required Qualifications:

  • Must have valid Drivers License access to an insured safe and reliable vehicle
  • Must pass a DHS NetStudy 2.0 background check.
  • Must be able to attend home visits and apartment showings with participants including visiting units that may only be accessible by climbing stairs.
  • Must possess an intermediate knowledge of Microsoft products including Word and Excel

Preferred Qualifications:

  • One year casework or similar experience such as PCA resource navigation etc.
  • Two years experience working with participants experiencing homelessness.
  • Fluency in language(s) used by our participants (Spanish Amharic Oromo Somali ASL)
  • Familiarity with community resources relevant to our participants needs

This position requires use of your own personal vehicle and will require insurance liability coverage of at least one hundred thousand dollars per person and three hundred thousand dollars per occurrence and must have a valid Drivers License and pass a driving record background check.
