Job Summary
Job Title: Registered Nurse Home Health / Hospice
Location: Gunnison CO
Contract Duration: 13 Weeks
Shift: Day Shift with Call Requirements
On-call: 12 nights per week and 12 weekends per month
Compensation:
Weekly Gross Pay: $1800 $1900 (based on experience)
Job Overview:
We are seeking an experienced Registered Nurse (RN) to provide compassionate high-quality care to patients in home health hospice and palliative care settings. This role involves direct patient care case management and coordination of services to ensure optimal outcomes in a community-based environment.
Key Responsibilities:
Provide skilled nursing care to home health hospice and palliative care patients
Conduct initial patient assessments and ongoing reassessments
Develop implement and update individualized Plans of Care
Perform case management and coordinate multidisciplinary services
Administer specialized nursing treatments and procedures
Monitor patient conditions and communicate changes to physicians and care teams
Educate patients and families on disease management and care plans
Maintain accurate clinical documentation and progress notes
Supervise and support other nursing staff as needed
Required Certifications & Licensure:
Active Registered Nurse (RN) License (Colorado or Compact)
Basic Life Support (BLS)
Colorado Adult Protective Services (CAPS) check required
Required Experience:
Minimum 3 years of nursing experience
Prior experience in Home Health and/or Hospice care (required)
Key Skills:
Strong patient assessment and clinical decision-making abilities
Experience with case management and care coordination
Ability to work independently in a home-based care environment
Excellent communication and patient education skills
Work Environment:
Community-based care setting
Requires travel to patient homes
On-call responsibilities as part of the role
