Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment in Valley City ND.
This role involves providing therapy services to patients in their homes across a 60-mile radius treating a mix of post-surgical fall recovery and general home health cases.
Key Responsibilities

Conduct in-home patient evaluations and develop care plans

Provide skilled PT treatment to home health and hospice patients

Manage a caseload of 57 patients per day

Document care using HomeCare HomeBase EMR
-
Educate patients and families on mobility safety and recovery
-
Coordinate care with interdisciplinary teams
Requirements
-
Active North Dakota PT License
-
Minimum 1 year of Home Health experience REQUIRED

BLS Certification (AHA)
-
Valid Drivers License & Auto Insurance REQUIRED

Strong time management and independent work skills
Assignment Details

Schedule: MondayFriday 8:00 AM 5:00 PM
-
40 hours guaranteed
-
No weekends or holidays
-
Coverage area: 60-mile radius
-
No interview required (fast submission process)
Important Notes
-
Must live 50 miles from facility
-
Cannot have worked with CommonSpirit/CHI/Dignity within past 12 months
-
Facility may cancel up to 4 shifts per contract

RTO must be submitted upfront
