Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a rehab-focused hospital in Highlands NC.
This role offers a diverse caseload across acute care swing bed skilled nursing and outpatient settings working within a highly collaborative rehab team.
Key Responsibilities

Perform patient evaluations and develop individualized treatment plans

Provide therapy across multiple settings (acute SNF outpatient)
-
Collaborate with PT/OT/SLP team on patient care and discharge planning
-
Participate in care conferences and interdisciplinary rounds
-
Document care using Cerner and NetHealth systems
-
Educate patients and families on treatment and recovery
Requirements
-
Active North Carolina PT License
-
CPR Certification (Required)
-
Minimum 3 years of medical/hospital experience preferred
-
Experience in hospital and/or nursing home settings REQUIRED
-
Strong communication and team collaboration skills
Assignment Details

40 hours per week

Floating required between rehab settings

Patient ratio: 1:1 care

Facility includes:

24 acute/swing beds
-
40 nursing home beds (5-star facility)
-
10 skilled beds
-

Access to multiple rehab gyms outdoor therapy space (expanding)
