Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment covering the Berkeley / Oakland area.
This is a field-based role working directly with patients in their homes focusing on improving functional mobility and independence.
Key Responsibilities

Perform in-home patient evaluations and develop treatment plans

Deliver skilled physical therapy services in a home health setting

Complete accurate and timely OASIS documentation
-
Monitor patient progress and adjust care plans accordingly

Educate patients and caregivers on exercises and safety

Maintain strong communication with interdisciplinary team
Requirements
-
Active California PT License (or pending with application)

Minimum 1 year of Home Health experience REQUIRED

OASIS experience REQUIRED
-
BLS Certification (AHA)
-
Valid drivers license and reliable transportation
Assignment Details

Schedule options: TuesSat or WedSun

25 visits 40 hours guaranteed

Mileage reimbursed (IRS standard)
