Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment covering Palmdale and Lancaster CA.
This role focuses on delivering one-on-one care in a home health setting helping patients improve mobility function and independence in their own environment.
Key Responsibilities

Evaluate and treat patients in a home health setting

Develop and implement individualized treatment plans

Perform functional mobility training and therapeutic interventions
-
Complete OASIS documentation and maintain accurate records
-
Educate patients and families on care plans and home exercises
-
Coordinate care with interdisciplinary teams
Requirements
-
Active California PT License (or pending at submission)
-
BLS Certification (AHA)
-
Minimum 1 year of Home Health experience
-
OASIS experience preferred (required or strong HH experience considered)

Valid drivers license and reliable transportation
Assignment Details
-
Schedule options: TuesSat or WedSun
-
25 visits 40 hours guaranteed
-
Mileage reimbursed (IRS standard)

Locals accepted at travel rate

Coverage area: Palmdale / Lancaster
-
On-call rotation may be required
