Job Summary
Lead Reablement Worker
Earn 13.21 per hour (25838 per annum) pro rata and great benefits including Health Cash Plan
Permanent full time (37.5 hpw) working shifts (07:00 - 19:30 or 09:00 - 21:30) taking turns on covering weekends and bank holidays

We support our customers with range of support needs which may include frailty and dementia we help them regain or maintain their independence and live well in their own homes. Some of our customers have more complex support needs and may require personal care including bathing toileting and getting dressed.
What youll do
We provide 24-hour support to our customers
- We have 82 apartments and our customers are 65 with Dementia or some care needs.
- Developing customer care plans coordinating reablement workers to deliver on the plans
- Leading on caseloads risk assessments regular reviews
- Working collaboratively with external agencies

You have
- A minimum Level 2 Diploma in Care willing to work towards Level 3 in Adult Care
- Experience working in supported housing
- An excellent understanding of safety risk management CQC requirements and legislation in regard to personal care
- Knowledge of local community and networks would be brilliant.
- To get from A to B youll need a vehicle insured for business purposes. The great news is that well pay your mileage!

The practical bits
Working on a rolling rota (07:00 - 19:30 or 09:00 - 21:30) taking turns on weekends and bank holidays
- Able to use technology for creating and updating support plans.
- Youll need an Enhanced DBS check done and we pay for that.
