Job Summary
ATC Healthcare Services is seeking acompassionate and dedicated Social Workerto join our team inFarmville VA. The Social Worker will support individuals and families by providing counseling care coordination and access to community resources. This role focuses on improving clients overall well-being addressing social and emotional challenges and helping individuals achieve stability and independence.
Key Responsibilities
Conductclient assessmentsto identify social emotional and environmental needs
Develop and implementindividualized care and support plans
Providecounseling and crisis interventionto individuals and families
Connect clients withcommunity resources housing healthcare services and support programs
Collaborate withhealthcare providers case managers and community organizations
Maintainaccurate case documentation and reportsin accordance with organizational policies
Advocate for clients to ensure they receiveappropriate services and support
Monitor client progress and adjust care plans as necessary
Qualifications
Bachelors or Masters degree in Social Work (BSW/MSW)or a related field
Active state licensure(if required in Virginia)
Previous experience incase management counseling or community support servicespreferred
Strongcommunication empathy and problem-solving skills
Ability to work effectively with individuals fromdiverse backgrounds
