Job Summary

Minimum Qualifications

Education: Completion of an accredited Licensed Practical Nurse (LPN) program.

Experience: At least one (1) year of experience providing services to individuals with mental disabilities.

Certifications/Licenses:

  1. Valid New York State Licensed Practical Nurse (LPN) license
  2. Current New York State registration
  3. Must maintain current First Aid and CPR certification

Work Eligibility: Must be authorized to work in the U.S.

About the Role

Under the supervision of a Supervising Registered Nurse the Licensed Practical Nurse provides nursing care in accordance with recognized nursing techniques procedures and established standards within the scope of practical nursing. The role includes case finding health teaching health counseling and the provision of supportive and restorative care under the direction of a registered professional nurse.

Primary Responsibilities

  • Provide symptom assessment management and supportive counseling to consumers
  • Administer medications and document activities in accordance with agency medication administration protocols; maintain copies of provider prescriptions
  • Assist in the formulation of nursing diagnoses and treatment plan recommendations including short- and long-term goals as part of an interdisciplinary team
  • Assess consumers health conditions daily and communicate changes in needs and plans of care to team members
  • Implement physician orders accurately both directly and indirectly
  • Report consumer needs problems and health observations to the Registered Nurse and/or physician
  • Observe and document consumer behavior and responses to treatment rehabilitation programs support services and medications in accordance with agency and audit standards
  • Document significant behavioral responses and assist in crisis and emergency situations to protect consumers staff and others
  • Perform weekly or assigned inventories of medical supplies and maintain accurate counts of controlled substances
  • Maintain medication records on a monthly or assigned basis
  • Participate in fire drills headcounts fire-watch and heat safety activities; assist with accounting for residents/consumers and missing person procedures as required
  • Maintain accurate and timely case records including progress notes service plan reviews assessments medical records and daily logbook entries
  • Provide crisis assessment and intervention
  • Assist consumers with basic daily living activities and support social activities and relationships
  • Provide or arrange emergency medical care
  • Conduct or assist with educational discussions on medication effects coping skills assertiveness training sexuality and related topics
  • Provide health-related referrals and community resource information
  • Participate in staff development activities
  • Ensure the medication room is clean organized and maintained
  • Perform other related duties as assigned
