The practice is based on competent clinical knowledge and focuses on clinical practice education collaboration consultation and research related to palliative care.
GENERAL RESONSIBILITIES
Under general direction and supervision of the Hospice Medical Director/CMOprovidesadministrative andpatientcareservices.
Position is Fulltime.
ESSENTIALFUNCTIONS
- Consistentwith42C.F.R.418.64(a)Physicianacknowledgesthathe/sheisalongwiththeHospiceMedicalDirectorHospicesphysicianemployeescontracted physicians and each patients attending physician responsible for thepalliation and management of the terminal illness and conditions related to theterminalillnessof Hospicespatients.
- PhysicianwillperformadministrativeandpatientcareservicesasrequestedbyHospiceortheHospiceMedicalDirectorwhichmayincludebutarenotlimitedtotheservicesdescribedinthisdocument.
- Physicianasaphysicianmemberof thehospiceIDG willperformcertification/recertificationofterminalillnessconsistentwith42C.F.R418.22(c).
- Physician will participate in IDG/transdisciplinary team conferences todevelopandrevisepatientcareobjectivesandprocedures;provideexpertconsultation and participate in team decisions regarding appropriate settings andlevelsofcareforpatients;andcollaboratewithpharmacistsconcerningmedicationformularyandbestprescribing practices.
- Physician will perform face-to-face encounters as needed along withthe performance of other clinical aspects related to the Hospices patient careprogramincluding coordinationof admissionsanddischarges.
- Physicianwillprovidedirectmedicalcaretopatientsintheirhomesettingorinthehospiceresidentialfacilitywhichmayincludebeingthepatientsdesignatedattendingphysicianifrequestedbythepatientorthepatientsrepresentative.
- PhysicianwillkeepabreastofstandardsandregulationsattendtrainingsparticipateinQAPIandmeet allotherregulatoryandagencyrequirements.
- Physicianwilldevelopandmaintainpositiverelationshipswiththephysician community promoting hospice and palliative care; precepts medicaltrainees.
- PhysicianwillparticipateinHospicescommitteeworkasrequested.
- Physician will participate in and/or perform palliative care consultationsasrequested.
- If serving as the Physician Designee Physician will assume responsibility of the medical component of the Hospices patient care program pursuant to the Physician Designee Policy and the Appointment of Physician Designee will not assume the Physician Designee role without direction from theHospiceorthe HospiceMedical Director.
- Physicianwillperformotherdutiesasrequired.
REQUIRED EDUCATIONANDEXPERIENCE
- Current unrestricted licensure of Doctor of Medicine or Osteopathy.
- Must be able to obtain physician licensure in the State of Ohio at time of start of employment.
- MustmaintainavalidDEAlicenseandboardcertification.
- Initial board certification in a primary ABMS specialty. Preferred primary specialties of oncologyinternalmedicinefamilypractice hospitalist medicineorgeriatrics.
- Minimum 2 years of demonstrated work experience in hospice and/or palliative care or another similar setting managing the care of terminally ill patients.
- Experience caring for patients in a hospice inpatient unit setting preferred.
- Board-certification or Board-eligibility in Hospice and Palliative Medicine PREFERRED.
- Applicants currently participating in a qualified Hospice and Palliative Medicine Fellowship may apply.
- If Board Eligible Board Certification in Hospice and Palliative Medicine is required within 2 years of employment.
- Alternatively Hospice Medical Director Certification with high-level work experience in the field may be considered in lieu of fellowship training and certification.
- Reliable transportation a valid drivers license a good driving record and auto accident insurance at a level acceptable to the agencys insurer are also required.
KNOWLEDGE SKILLS ABILITIES
- Thorough knowledge of the principles and practices of hospice and palliativecare.

- Ability to lead a team for inpatient hospice care and Care Centers for Reserve Care
- Primary physician lead as inpatient rounder.
- Attending for residential patients as required.
- Weekend support as needed

PHYSICAL REQUIREMENTS
This is medium to heavy work requiring the exertion of up to 100 pounds of force occasionally and up to 20 pounds of force frequently. Work requires climbing balancing stooping kneeling reaching standing pushing pulling lifting fingering grasping and repetitive motions vocal communication hearing and visual acuity. The employee is subject to inside and outside environmental conditions hazard and atmospheric conditions.
