We are seeking a Registered Nurse (RN) Patient & Family Educator to:

  • Provide specialized education to adult and pediatric patients on conditions including diabetes heart failure anticoagulation therapy (Coumadin and Lovenox) and brief alcohol intervention to support safe and effective care.
  • Assess patient and family learning needs and deliver individualized education on essential self-management skills such as blood glucose monitoring insulin administration and continuous glucose monitor use.
  • Partner with interdisciplinary teams across inpatient units and clinics to coordinate and reinforce education and support patients and families in understanding their care plans.
  • Utilize evidence-based education strategies health literacy principles and innovative teaching technologies to enhance patient and family learning.
  • Serve as a resource and collaborator for clinical staff reinforcing patient education and supporting a comprehensive patient and family education program across UW Health.
  • Preferred candidates will have experience caring for patients with diabetes heart failure or anticoagulation therapy needs.

Qualifications
  • Bachelors Degree in Nursing Preferred
Work Experience
  • 3 years recent relevant clinical experience Required
  • Broad experience in medical/surgical and/or pediatric nursing Required
  • Demonstrated leadership experience Preferred
Licenses & Certifications
  • RN Licensure in the State of Wisconsin Upon Hire Required
  • BLS/CPR Certification Upon Hire Required
  • CDE Preferred
