Job Summary

Nurse Practitioners are encouraged to consider an available full-time position with Liberty Healthcare at DHCI - a skilled nursing facility located in Smyrna Delaware.

In this role you will focus on providing primary care to a case load of approximately forty older adults and people who are living with various physical and cognitive disabilities.

Your day-to-day responsibilities will include:

  • Daily patient evaluation through rounding on units
  • Conducting examinations performing diagnostic tests providing preventive care developing and managing treatment plans for acute and chronic diseases and prescribing medications
  • Providing consultation and health education to nursing and ancillary staff
  • Collaborating with the facilitys Medical Director nursing team and ancillary staff
  • Actively participating in multidisciplinary treatment team meetings
