Shift: Tuesday-Saturday 8am-4pm (40 hrs/week on-call rotation required)
Weekly Pay: $2720
Job Description
Seeking a Travel Home Care RN to provide high-quality nursing care for patients in a home hospice setting. This role requires strong clinical judgment case management experience and the ability to work independently while coordinating care with the healthcare team.
Key Responsibilities

Conduct patient assessments and develop individualized care plans
-
Serve as Case Manager for assigned patients and manage caseloads efficiently
-
Provide hospice care including high-tech skills (IV therapy wound VAC complex wound care)
-
Collaborate with LPNs and interdisciplinary team members to ensure care plan compliance
-
Maintain accurate and up-to-date documentation in the EMR (McKesson preferred)
-
Educate patients and caregivers on treatments medications and discharge instructions
-
Coordinate safe patient discharges and transfers
-
Participate in peer review audits and case conferences
-
Advocate for patient care and ensure quality care sensitive to patient/family needs
Qualifications

Current Vermont RN license (full unrestricted)

Minimum 2 years clinical experience preferred

Hospice/home care experience required
-
Strong communication organizational and critical thinking skills
-
Ability to work independently and respond to on-call patient needs
