Salary Range:  $43.47 to $54.34
Hours: Anywhere between 0 - 37.5 hours per week
Shifts: Within 8.00am - 4.00pm on Weekdays and Weekends
Status: Temporary Full-Time
Closing Date: March 20 2026

Duties

  • Co-lead the modernization and rollout of the HELP program across acute care units.
  • Develop workflows tools education and volunteer training to support delirium prevention and functional preservation for older adults.
  • Contribute to Older Adult Action Plan initiatives clinical pathway redesign and QI/evaluation work.
  • Serve as an expert resource on delirium prevention and elder-friendly care practices.
  • Provides assessment and treatment of acute care in-patients at the Toronto Western Hospital
  • Provides evidence-based and patient centered practice to patients seen
  • Supports the development and implementation of best practice models of occupational therapy in the acute care setting at TWH
  • Works with multidisciplinary teams
  • Participates in the clinical education of occupational therapy students
  • Participate in Allied Health meetings and quality initiatives
  • In cooperation with other members of the occupational therapy in-patient team provides assessment and treatment for patients across the hospital as required.

Qualifications :

  • Masters of Science degree in Occupational Therapy or equivalent
  • Current registration with the College of Occupational Therapists of Ontario (COTO)
  • Canadian Association of Occupational Therapists membership preferred
  • Previous experience in an acute care Hospital environment preferred
  • Knowledge of geriatric syndromes delirium prevention and functional assessment.
  • Interest in and/or experience with program development QI and change management.
  • Experience training and working with volunteers.
  • Strong communication skills and the ability to collaborate effectively across interprofessional teams.
  • Montreal Cognitive Assessment (MoCA) certification preferred
  • Excellent interpersonal time management and organizational skills
  • Must be physically capable of performing the daily activities associated with patient care and occupational therapy treatment
  • Experience in assessment and treatment of general internal medicine patients Ortho/Msk patients and patients with Neurological deficits
  • Demonstrated appropriate and effective written oral and nonverbal communication skills
  • Computer literacy
