Salary Range: $43.47 to $54.34
Hours: Anywhere between 0 - 37.5 hours per week
Shifts: Within 8.00am - 4.00pm on Weekdays and Weekends
Status: Temporary Full-Time
Closing Date: March 20 2026
Duties
- Co-lead the modernization and rollout of the HELP program across acute care units.
- Develop workflows tools education and volunteer training to support delirium prevention and functional preservation for older adults.
- Contribute to Older Adult Action Plan initiatives clinical pathway redesign and QI/evaluation work.
- Serve as an expert resource on delirium prevention and elder-friendly care practices.
- Provides assessment and treatment of acute care in-patients at the Toronto Western Hospital
- Provides evidence-based and patient centered practice to patients seen
- Supports the development and implementation of best practice models of occupational therapy in the acute care setting at TWH
- Works with multidisciplinary teams
- Participates in the clinical education of occupational therapy students
- Participate in Allied Health meetings and quality initiatives
- In cooperation with other members of the occupational therapy in-patient team provides assessment and treatment for patients across the hospital as required.
Qualifications :
- Masters of Science degree in Occupational Therapy or equivalent
- Current registration with the College of Occupational Therapists of Ontario (COTO)
- Canadian Association of Occupational Therapists membership preferred
- Previous experience in an acute care Hospital environment preferred
- Knowledge of geriatric syndromes delirium prevention and functional assessment.
- Interest in and/or experience with program development QI and change management.
- Experience training and working with volunteers.
- Strong communication skills and the ability to collaborate effectively across interprofessional teams.
- Montreal Cognitive Assessment (MoCA) certification preferred
- Excellent interpersonal time management and organizational skills
- Must be physically capable of performing the daily activities associated with patient care and occupational therapy treatment
- Experience in assessment and treatment of general internal medicine patients Ortho/Msk patients and patients with Neurological deficits
- Demonstrated appropriate and effective written oral and nonverbal communication skills
- Computer literacy
