What you will do

Manage and coordinate your patient caseload
Complete patient assessments and follow up visits
Develop and update individualized plans of care
Communicate changes in condition with the care team
Provide education and support to patients and families

What you need

Active RN license
At least 1 year of clinical nursing experience
Reliable transportation and willingness to travel within the service area
Ability to lift up to 50 pounds
Home Health or Hospice experience preferred
Experience with HomeCare HomeBase preferred
