Job Title: HIM Analyst I
Location: Philadelphia PA 19104/Remote
Note: Position is mostly remote with contractors needing to come onsite occassionally for mandatory meetings required by leadership.
Summary of Job:
The HIM Documentation Auditor is responsible for auditing monitoring and reporting clinical documentation to ensure compliance with regulatory standards set by the Centers for Medicare & Medicaid Services (CMS) The Joint Commission (JC) and the Pennsylvania Department of Health (DOH). This role reviews electronic health records monitors scanned documentation and transcription integrations and supports system workflows within Epic and ChartMaxx. The position also assists with data integrity compliance monitoring and issue resolution related to health information systems.
Must-have/Required skills:
- High School Diploma or GED
- At least 1 year of experience in Health Information Management (HIM) or a related field
- Knowledge of Microsoft Office
- Strong analytical and problem-solving skills
- Strong attention to detail and critical thinking abilities
- Ability to work independently and within a team environment
- Good communication and customer service skills
- Strong organizational and time management skills
- Ability to maintain confidentiality and comply with healthcare information security policies
Nice-to-Have Skills:
- Bachelors degree in Health Information Management or related field
- RHIA (Registered Health Information Administrator) certification
- RHIT (Registered Health Information Technician) certification
- Experience with Epic Electronic Health Record (EHR) systems
- Experience with ChartMaxx or M Modal transcription systems
- Knowledge of healthcare regulatory standards (CMS Joint Commission DOH)
Key Responsibilities and Duties:
- Clinical Documentation Auditing
- Conduct retrospective chart reviews to ensure compliance with federal state and accreditation standards
- Verify accuracy and integrity of scanned records within Epic and ChartMaxx
- Ensure compliance with healthcare documentation and record management policies
- System Monitoring & Data Integrity
- Monitor interfaced transcription reports and scanned document links between systems
- Ensure patient identifiers are consistent across Epic ChartMaxx and transcription systems
- Address and resolve integration issues such as name changes aliases and record merges
- Workflow & System Support
- Support Epic Release to Inspector functionality for approved enterprise record audits
- Monitor and manage failed fax distributions of discharge summaries to physicians and providers
- Manage HIM ServiceNow work queues including ChartMaxx account creation and Epic scan deletions
- Security & Compliance
- Monitor FairWarning alerts and notify users whose accounts are flagged
- Ensure compliance with enterprise information security policies and standards
- Support activities related to regulatory compliance and patient information confidentiality
- Collaboration & Issue Resolution
- Work with hospital record custodians to support record release processes
- Collaborate with Information Systems (IS) support teams to resolve system or software issues
- Assist in managing the lifecycle of paper and electronic health records
- Additional Responsibilities
- Support information security integration in technology systems handling patient data
- Assist with documentation management tasks and special assignments as required
Work Environment:
- Healthcare or hospital information management setting
- Interaction with clinical staff IT teams and administrative personnel
- Work involves handling confidential patient health information and electronic health records (EHR)
