What Youll Do

Comprehensive Outpatient Care

• Conduct mental health assessments and develop individualized treatment plans.
• Provide outpatient therapy to adults and/or children.
• Deliver crisis intervention and behavioral stabilization as needed.
• Maintain a structured caseload of one patient per hour to ensure quality care.

Integrated Team Collaboration

• Work side-by-side with Primary Care Providers within our MindPATH model.
• Collaborate with care managers and interdisciplinary teams.
• Connect patients with appropriate community and behavioral health resources.

Clinical Documentation & Monitoring

• Maintain accurate treatment plans and progress notes.
• Monitor patient outcomes and adjust interventions as needed.
• Prepare discharge planning and case summaries.

What You Bring

Required:

• Masters Degree in Social Work Counseling or related field.
• Active LPC or LMSW license.
• Minimum 3 years of experience providing outpatient behavioral health services.
• Experience with assessments treatment planning therapy and crisis intervention.

Preferred Qualities:

• Comfortable with structured outpatient medication-management collaboration.
• Values team-based integrated care.
