The University of Miami Uhealth Department of Ophthalmology General / Comprehensive at Bascom Palmer in SoLé Mia has an exciting opportunity for anAdvanced Practice Registered Nurse (APRN). The incumbent delivers medical care to a wide variety of patients. TheAPRNis alsoexaminesand treats patients independently and in autonomous collaboration with other health care professionals. Ensures proper illness and injury care and disease prevention diagnosis treatment and recovery. May prescribe medications and order diagnostic care.

CORE JOB FUNCTIONS

  • Performs and documents complete physical examinations and comprehensive health histories.

  • Functions independently to perform age-appropriate history and physical for patients.

  • Orders and interprets diagnostic and therapeutic testsrelativeto patients age-specific needs.

  • Prescribesappropriate pharmacologicand non-pharmacologic treatment modalities

  • Implements interventions to support the patient to regain ormaintainphysiologic stability.

  • Assistswith the provision of carein accordance withfacility state and federal regulations.

  • Monitors the effectiveness of interventions.

  • Facilitates the patients transition within and between health care transferring and discharging patients.

  • Collaborates with multidisciplinary team members by makingappropriate referrals.

  • Facilitates staffpatientand family decision making by providing educational tools.

  • Adheres to University and unit-level policies and procedures and safeguards University assets.

COREQUALIFICATIONS

Education:

  • Masters degreein relevant field

Certification and Licensing:

  • Valid State of Florida APRNLicense

Experience:

  • Minimum 2 years of relevant advanced practice nursing experience preferred

  • Ability to exercise sound judgment in making critical decisions.

  • Skill in completing assignments accurately and with attention to detail.

  • Ability to analyzeorganizeand prioritize work under pressure while meeting deadlines.

Department Specific Functions

The APRNwill perform a patient assessment to include medical history and physical exam and in collaboration with the physician mentor and develop an appropriate plan of care in the direct management of patients with severaleye conditions includingneuro-ophthalmology and glaucoma as well as exploring areas ofconsult servicesocular surface services oculoplastic and tele-ophthalmology/virtual care. TheAPRNworks directly with staff administration patients and families to ensure quality care across the continuum as an expert in selected practice areas. TheAPRNintegrates current knowledge of professional issues trends in healthcareand technological advances into the scope of practice. Additionallyhe/sheparticipatesin the collection of researchdata quality improvement dischargeplanningand coordination of patient care.

APRN role responsibilities within scope of license include:

  • Follow non acute chronic neuro patients that need continued care

  • Follow non acute (cornea retina glaucoma) patients that need continued care

  • Facilitate visual fields refractions testing etc.

  • Facilitate referrals to subspecialties and other referral requests.

  • Route international patients and second opinion requests

  • Answer patient questions emails

  • Review records in advance of physician review

  • Complete telehealth visits

  • Measure prisms

Additionally duties and responsibilities of this position include but are not limited to:

Accountabilities:

I. Clinical Role

  • Provides care to a select group of patients bydeterminingandrenderingadvanced nursing and medical interventions.

  • Assessment/Diagnosis: Assesses patients by interviewing/examining the patient reviewing the patients medical history physicians diagnosis and orders diagnostic test results todeterminemedical diagnosis plan of care andmonitorpatients progress.

  • Performs physical assessment.

  • Reviews the medical record including physician/consultation noteslaband diagnostictestsresults. Formulates medical diagnoses based on the above data.

  • Collaborates with physicians through discussion and assessment of diagnoses.

  • Planning/Outcomes: Develops a plan of care from the assessment/diagnoses and physician collaboration.

  • Identifiesexpected outcomes based on the assessment/diagnoses and physician collaboration.

  • Includes interventions in plan of care that reflects expected outcomes.

  • Documents and coordinates plan of care.

  • Providespatient/family educationregardingdisease processes treatment options treatment outcomespossible complications medications dischargeplanningand lifestyle modification.

  • Integrates current knowledge of professional issues trends in healthcareand technological advances into scope of practice.

  • Implementation: Institutesnecessary actions as outlines in the plan of care.

  • Collaborates with team members to ensure implementation ofplanas described.

  • Implements physician orders perprotocols.

  • Communicates plan of care to team members.

  • Assists in the management of family dynamics and coping mechanisms during patient care.

  • Engagesin performance improvement activities to support the mental health needs of our patient population.

  • Evaluation: Evaluates the patients progress inattainingexpected outcomes byfollow-upwithcollaboratingphysician.

  • Evaluates aswarrantedby patients condition daily by assessment of physical findingslaband diagnostic test results.

  • Formulates new plan of care asdeterminedby evaluation.

  • Consultation/Collaboration: Consults with collaborating physician(s) on implementation evaluation and revision of treatment plan.

  • Coordinates care with interdisciplinary healthcare team in the development and implementation of plan of care.

  • Facilitatescommunication between the nursing staff and physicians with leadership skills and on-unit mentoring.

  • Collaborates with members of the education department and units leadership team in planning educational opportunities and competency evaluation for nursing staff members.

  • Conducts orparticipatesin multidisciplinary rounds and ensuresappropriateand quality care for patients.

  • Rotates and refers to support services as needed.

II. Research

  • Initiates and/orparticipatesin hospital research activities.

  • Utilizes researchfindingand nursing theory in clinical practice.

  • Promotes evidence-based/knowledge-based nursingpractice.

  • Collects data for patient and program evaluation andparticipatesin quality assessment and improvement activities.
