- Participate with assigned transitional service providers and coordinate youth schedules as appropriate (Provides information and assists in accessing services/programs).
- Meets with youth service providers family and group leaders (when appropriate) at a minimum of once monthly.
- Administer and update life skills assessments as necessary. Throughout reintegration process focus on both protective and high-risk factors. Collaborate with family members and/or youths support system to reduce barriers for success after residential treatment.
- Provide referrals to community resources and utilizes communication techniques to develop and maintain a collaborative alliance with community partners and organizations as assigned.
- Provide support and coaching to youth utilizing a solution-focused approach that assists in the development and support of life skills.
- Participate in collaborative case conferences to provide information and recommendations according to program expectations.
- Act as an advocate for youth and provide youth with community resources.
- Complete all documentation relevant to progress of the youths transitional case plan including but not limited to case notes housing/employment applications college admissions & required Michigan Rehabilitation Services & Michigan Youth Re-Entry Initiative paperwork.
- Collect additional background information as requested and updates youth/agency records where appropriate. This may include but not limited to birth certificate Social Security number previous professional supports advocacy of new needs/services as identified and requesting release of info as appropriate
- Collect intake and referral information for new youth and document progress. Monitor and adjust the youths case plan when applicable.
- Identify and report the needs of youth to the supervisor to ensure appropriate service will be identified and addressed.
- Participating with Youth Advisory Committee as requested by youth offering support with advocacy. Meetings are scheduled & held twice monthly.
- Provide supportive services to youth in community settings. This may include accompanying youth to court appearances appointments and various volunteer and work-related opportunities in the community.
- Coordinate school records upon request.
- Coordinate with local community colleges and assist youth in requesting financial assistance.
