Licensed Practical Nurse (LPN) Home Health Valparaiso IN

What You Will Do

Provide skilled nursing care under the direction of an RN or physician
Follow and verify the patient plan of care during each visit
Document skilled nursing visits and patient progress
Communicate changes in patient condition to the care team
Educate patients and caregivers on self care and safety

What You Need

Active LPN license in the state of Indiana
At least one year of clinical nursing experience
Home health or hospice experience preferred
Reliable transportation valid drivers license and insurance
Ability to lift push pull or support up to 50 pounds
