Monday - Friday 8:30 AM - 5:00 PM

What you bring to SBHG:

Bachelors degree in Mental Health

Two years direct service experience with the mentally ill preferably adults preferred

Bilingual in Spanish preferred.

Valid California Drivers License.

How you will make a difference:

  • Responsible for helping residents meet service and resource needs including housing vocational/educational services transportation scheduling appointments and other social support services.
  • Responsible for identifying housing resources and developing relationships with community agencies.
  • Assists clients in completing facility consent forms upon admission
  • Actively participates in treatment team meetings and is responsible for working with other disciplines to develop a coordinated treatment plan for assigned clients.
  • Provides consultation as needed with families and the Case Manager on issues regarding vocation education and housing.
  • Documents social services that are provided and records them in the patient chart.
  • Assists with transportation to appointments to provide direct linkage to resources as needed

Division/Program Overview

The TAY Crisis Residential Treatment (CRT) facility is a short-term voluntary residential facility. Designed to help Transitional Age Youth (TAY) ages 18-25 who experience a psychiatric crisis to safely stabilize recover and transition back to community.

Services include:

24/7 program

Therapeutic and mental health services

Rehabilitation and recovery services (including substance rehabilitation)

Pre-vocational or vocational counseling

Medication evaluation and support services

Daily exercise and health/wellness education

Crisis intervention.

In accordance with California law the grade for this position is 18.88 - 28.32. Placement within the grade is determined based on experience internal equity and other factors permitted by law.
