What you bring to SBHG:

Bachelors degree in Mental Health

Two years direct service experience with the mentally ill preferably adults preferred

Bilingual in Spanish preferred.

Valid California Drivers License.

How you will make a difference:

Responsible for helping residents meet service and resource needs including housing vocational/educational services transportation scheduling appointments and other social support services.
Responsible for identifying housing resources and developing relationships with community agencies.
Assists clients in completing facility consent forms upon admission
Actively participates in treatment team meetings and is responsible for working with other disciplines to develop a coordinated treatment plan for assigned clients.
Provides consultation as needed with families and the Case Manager on issues regarding vocation education and housing.
Documents social services that are provided and records them in the patient chart.
Assists with transportation to appointments to provide direct linkage to resources as needed

Division/Program Overview

The TAY Crisis Residential Treatment (CRT) facility is a short-term voluntary residential facility. Designed to help Transitional Age Youth (TAY) ages 18-25 who experience a psychiatric crisis to safely stabilize recover and transition back to community.

Services include:

24/7 program

Therapeutic and mental health services

Rehabilitation and recovery services (including substance rehabilitation)

Pre-vocational or vocational counseling

Medication evaluation and support services

Daily exercise and health/wellness education

Crisis intervention.

In accordance with California law the grade for this position is 18.88 - 28.32. Placement within the grade is determined based on experience internal equity and other factors permitted by law.
