Job Summary
We are seeking a dedicated Occupational Therapist (OT) to provide rehabilitation services in a Skilled Nursing Facility (SNF) setting. The Occupational Therapist will evaluate and treat patients recovering from illness injury or surgery focusing on restoring functional independence and improving the ability to perform activities of daily living.
Key Responsibilities

Evaluate patients physical cognitive and functional abilities to develop individualized treatment plans.

Provide therapeutic interventions to improve mobility coordination strength and daily living skills.

Implement rehabilitation programs that help patients regain independence and improve quality of life.

Educate patients and caregivers on adaptive techniques safety practices and the use of assistive devices.

Monitor patient progress and adjust treatment plans as needed to achieve rehabilitation goals.

Document patient assessments treatments and outcomes in accordance with facility policies and regulatory requirements.

Collaborate with physicians nurses physical therapists speech therapists and other healthcare professionals to coordinate patient care.

Participate in interdisciplinary care planning and discharge planning as appropriate.
Qualifications

Active Occupational Therapist (OT) license in the state of practice.

Minimum of one year of occupational therapy experience preferably in a skilled nursing or rehabilitation setting.

Graduation from an accredited Occupational Therapy program.

Strong clinical assessment communication and patient care skills.

Ability to work independently and as part of a multidisciplinary healthcare team.
