Job Title:RN SupervisorHome Health
Job Location:Dunmore PA / Lackawanna andLuzerne County
Shift:Full-time

Job Duties / Responsibilities include butare not limited to the following:

• Demonstrateexceptional professionalismandcare coordination viathoroughcommunication withteam members patients and family / caregivers physicians referral sourcespayer sources
• Directdaily office clinical activitiesincluding patient schedulingand triaging calls fromphysiciansemployees andpatients
• Reviewclinical records and documentation to verify regulatory and agency compliance including infectioncontrol standardsevaluation of infection control logsidentify and addresstrends related to patient infections
• Identify and discussoperational issues keepingstaff informed of additions or changes in policies
• Participates in performance improvement activities and provides backup support to staff
• Maintains current practice knowledge through internal and external training programs / seminars and acts as a resource for the interpretation of clinical practice / standards
• Assist withrecruitment interviewing selection and orientation process ofnew team members
• Participatein community events such as health fairsconventions and sales calls to provide clinical expertise
• Evaluateperformance related to job goals and requirements provide feedback
• Participatein special projects and performs other duties as assigned

Requirements:

• Current license as a Registered Nurse in Pennsylvania
• Home Health experience is A MUST
• Excellent assessment anddocumentation skills and ability to meet deadlines
• Strong interpersonal andleadership skills with a positive pro-active attitude
• Ability to work independently as well ascollaborate with team members
• Excellent verbal /written communication time management andorganizational skills
• Must have a current drivers license
• Ability to travel to case assignments PRN
