Job Summary
Squirrel Hill Health Center is hiring a Substance Use Disorder (SUD) Program Coordinator to support recovery improve health outcomes and sustain public health initiatives like harm reduction and medication-assisted treatment (MAT). This role serves as a liaison between patients providers staff and community partners while coordinating all aspects of the SUD program.
Responsibilities:
- Coordinate the SUD/MAT program and track patient progress.
- Conduct patient assessments create care plans and provide education on harm reduction and overdose prevention.
- Facilitate referrals to behavioral health counseling and social services.
- Maintains program standards of COE MAT and SUD programming including workflows and documentation for providers and patients.
- Collects tracks and reports program data in REDCap and other reporting systems. Prepares reports for MCOs PittPERU funders and accrediting agencies.
- Support Mobile Unit visits including check in and appointment scheduling.
- Build relationships with external providers and agencies to support patient care.
Qualifications:
- Bachelors degree in social work public health counseling nursing or related field (MSW preferred).
- Experience in substance use disorders or behavioral health preferred.
- Knowledge of MAT harm reduction medical terminology and EHR systems.
- Strong organizational communication and documentation skills.
- Proficiency in Microsoft Office/Google Suite; REDCap experience preferred.
- Bilingual ability is a plus.
- Commitment to serving underserved populations and aligning with the SHHC mission.
