Summary of Duties
To provide a full range of social work services to patients and their families admitted to D5 primarily for Neurosurgery as well as Orthopedic Surgery in the Tory Trauma Program. This includes psycho-social assessments crisis intervention counselling to patients and families discharge planning linkage to community resources end of life care Advance Care planning team consultation and collaboration.
Qualifications/Skills
- Masters of Social Work degree required
- Member of the Ontario College of Social Workers and Social Service Workers in good standing
- Minimum two (2) years of experience working in a Hospital setting preferably in trauma
- Exceptional communication skills required
- In-depth understanding of the Sunnybrook Discharge Policy and its application
- Knowledge of the HCCA and SDM act
- Familiarity with issues of capacity and consent
- Strong negotiation and conflict resolution skills
- Ability to be a team player as well as self-directed
- Ability to manage multiple tasks and set priorities in a fast-paced environment
- Commitment to student education
