SUMMARY
The LPN MDS role is responsible for various tasks related to resident assessments and care planning including collecting resident data collaborating with interdisciplinary teams and contributing to care planning processes to enhance the quality of resident care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Conducts required initial quarterly annual and significant change assessments based on Resident Assessment Instruction (RAI) guidelines to ensure compliance with state and federal standards.
- Collects analyzes and documents data about each residents health condition and medical history.
- Develops revises and reviews person-centered care plans as necessary.
- Communicates frequently with residents and their representatives and coordinates care plan meetings.
- Collaborates and effectively communicates with nursing staff and facility personnel to coordinate quality care for residents.
- Attends Staff/Unit meetings promoting developing and fostering a positive working environment.
- Functions as an integral part of the Quality Improvement Program and contributes to improving the facilitys overall quality of care provided to residents.
- All other duties as assigned.
QUALIFICATIONS
EDUCATION and/or EXPERIENCE
To perform this job successfully an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge skill and/or ability required.
- Associate degree or equivalent from a two-year college or technical school and minimum 2 years of experience in the field.
- Must have prior experience with MDS assessments and care
- Certified as a Resident Assessment Coordinator.
- Strong assessment and Clinical skills.
- Proficient in electronic health record systems and MDS software.
- Effective communication and interpersonal skills.
CERTIFICATES LICENSES REGISTRATIONS
- Valid State of Florida LPN License and BLS CPR Certification.
