Compensation:
Compensation is structured at $150 per visit with no limit on the number of visits completed while on call.
This is a flexible per-visit opportunity ideal for professionals seeking autonomy and the ability to increase earnings based on availability and responsiveness.
Your Role:
As a Care Coordinator at Family Best Care you will play a pivotal role in ensuring seamless service delivery and exceptional client experiences. You will serve as the central point of coordination between clients caregivers and the internal team.
Your responsibilities will include:
- Conducting initial client intakes and ongoing non- clinical reassessments
- Developing and updating detailed care plans based on client needs and family input
- Communicating clearly and professionally with families regarding care
- Maintaining and completing accurate assessment documentation
- Ensuring compliance with agency policies and Washington State home care regulations
- Supporting caregivers with clear expectations and accountability standards
- Monitoring service quality and proactively addressing concerns
Requirements:
- Prior experience in home care coordination client intake healthcare operations or case management preferred
- Strong organizational and problem-solving skills
- Excellent documentation and communication abilities
- Ability to work independently and manage multiple priorities
- Professional compassionate and solution- oriented approach
- High attention to detail and reliability
- Strong commitment to ethical conduct and accountability
