This position will assist our patients by utilizing a strong team-based approach to care with a high integration of APP and MDs for the clinic and inpatient and dialysis units for all 3 phases of nephrology care.
Desired skill set: Experience in complex chronic disease management
Typical Shift: up to 0.4 FTE per pay period
5% clinic work
5% hospital work
90% dialysis rounding
Ideal candidate does not need to live in the Twin Ports area; candidate could live in areas neighboring locations in Northwestern WI.
Schedule: Variable start and end times due to travel
Estimated Patients Per Day: 12 (office) 12-15 (dialysis)
Other responsibilities: Assist Physicians with non-encounter work
