Job Summary
We are currently seeking qualified candidates for a full time Long Term Care Liaison - LVN positionin the Clear Lakearea.
Job Description:
The Long Term Care Liaison LVN positionsworks closely with the Clinical team patients families and nursing facilities.
Qualifications:
- Graduate of an accredited college or university or two years of experience working in hospice and/or home health patient care
- Must be a Licensed Vocational Nurse (LVN) in the State of Texas
- Experience with long term care - required
- Must have good oral communication and writing skills
- Must be able to speak well in public
- Must be able to practice the principles of good public relations
- Must have a current state drivers license
- Must have reliable transportation
- Must have a current auto insurance policy
- Clean background check and drug screen
