Job Summary

Social Worker Enhanced Supportive Neighbourhood

The Social Worker in our Enhanced Supportive Neighbourhood provides emotional psychosocial and practical support to residents and families living within the Enhanced Supportive Neighbourhood (ESN). This role focuses on complex case management care transitions and family partnership to support residents with higher and changing needs.

Working as part of an interdisciplinary team the Social Worker supports resident well-being promotes dignity and belonging and contributes to successful transitions into and out of the ESN program in alignment with Schlegel Villages relationship-centred philosophy of care.

Qualifications

  • Degree in Social Work (BSW/MSW) or Diploma in Social Service Work with specialization in gerontology or a related field
  • Registration or eligibility for registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) preferred
  • Minimum 23 years experience working with seniors dementia care mental health or complex care populations preferred
  • Experience in long-term care retirement living or community health settings considered an asset
  • Strong interpersonal communication and documentation skills
  • Demonstrated commitment to person-centred and relationship-based care
  • Ability to work independently and collaboratively within interdisciplinary teams

Key Responsibilities

  • Provide emotional psychosocial and practical support to residents and families within the ESN neighbourhood with a focus on transitions adjustment complex case management and end-of-life care.
  • Support residents and families during admission health changes and care transitions helping them navigate complex needs and systems.
  • Participate in interdisciplinary care planning case conferences and ongoing collaboration with clinical recreation spiritual care and neighbourhood teams.
  • Advocate for resident dignity preferences and rights while supporting relationship-centred care.
  • Connect residents and families with appropriate internal and external community resources and supports.
  • Facilitate or support individual and small-group psychosocial and engagement programs.
  • Maintain accurate timely documentation in resident records including psychosocial observations and care planning input.
  • Identify emerging risks concerns or changes in resident well-being and support early intervention strategies.
  • Actively support and contribute to the planning and coordination of successful discharge from the ESN program including proactive communication with families care teams and community partners.
  • Contribute to a positive respectful and inclusive village culture through professional practice ethical conduct and ongoing learning.
