Your role and responsibilities

In this role you will support the effective delivery of the Brokerage service by:

  • Ensuring each team member has a clear tailored development plan aligned to the wider Business Plan and that progress is monitored efficiently and on time to support highquality performance evaluation.
  • Producing accurate performance data and reports to support strategic commissioning and contract managers contributing valuable insight and driving continuous improvement in our processes.
  • Working closely with Hospital Discharge Teams Social Work Services and HomeFirst colleagues to enable safe timely and wellcoordinated discharges.
  • Supporting the Brokerage Team to build and maintain positive relationships with internal and external customers ensuring excellent service delivery and identifying opportunities for service and process improvements.
  • Motivating teams to achieve strong outcomes by reviewing performance setting realistic objectives and encouraging a culture of accountability and success.

You will need

We are looking for someone who brings:

  • NVQ4 in Project Management (or an equivalent qualification/experience) or a willingness to work towards this.
  • Strong literacy and numeracy skills with excellent attention to detail.
  • Experience or knowledge of Residential and Nursing Care Homes and Supported Accommodation (desirable but not essential).
  • A proactive approach to learning and adapting to new and evolving systems processes and working practices.
  • Proven ability to work independently - planning prioritising and managing competing deadlines in a fastpaced pressurised environment.
