Human Touch Home Health is seeking a licensed Physical Therapist (PT) for a PRN (as-needed) home health position serving patients in Blacksburg and surrounding communities.

Position Overview
The Physical Therapist will provide skilled therapy services in the home setting supporting patients in improving mobility strength balance and overall function. This position requires independent clinical judgment and coordination with the care team.
Responsibilities
Perform in-home PT evaluations and reassessments
Develop and implement individualized treatment plans
Provide therapeutic interventions per physician orders
Complete documentation accurately and timely
Educate patients and caregivers on home exercise programs
Collaborate with physicians and interdisciplinary team members
Supervise PTAs as applicable
Qualifications
Active Virginia Physical Therapist license
Minimum 1 year of experience (home health preferred)
OASIS experience preferred
Strong clinical and documentation skills
EMR experience preferred
Valid drivers license and reliable transportation
This PRN position serves patients in Blacksburg VA and surrounding areas based on referral needs and clinician availability.
