Job Summary

Job Description:

  • Obtains a history and completes physical examinations of incarcerated patients.
  • Evaluates and treats patients as medically indicated in compliance with CHS policies procedures and Clinical Practice Guidelines.
  • Documents patient encounters according to CHS protocols in a SOAPE format in the progress notes or on the appropriate CHS clinical form.
  • Develops and updates individualized treatment plans for patients with chronic conditions or special needs.
  • Orders onsite lab work treatments and medications as medically indicated.
  • Refers to outside specialists outside diagnostic studies and other outside services as medically indicated.
  • Performs minor procedures such as sutures incisions drainage of abscesses simple splints punch biopsies and pelvic exams.
  • Participates in peer reviews in-service training quality improvement processes and other meetings necessary to improve patient care.
  • Maintains applicable licensure and skills according to the requirements by state law department policies and licensure agencies.
  • Supervises Physician Assistants as assigned by the Medical Director.


Requirements

Minimum Qualifications:

  • Diploma/transcripts to support graduation with a Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) from an accredited program.
  • Residency and/or Fellowship certificates to support completion of an accredited United States Residency or Fellowship training program in primary care field.
  • Current valid unrestricted physicians license to practice allopathic or osteopathic medicine issued by the Arizona Medical Board.
  • Current completed PSV from the Arizona Medical Board.
  • Current DEA/Controlled Substance Registration Certificate to prescribe Schedule II III and IV drugs.
  • Current completed NPI verification.
  • Current completed fit testing for N95 particulate respirators.
  • Current valid unrestricted Arizona Dispensing License issued by the Arizona Medical Board.
  • Minimum three years of past work experience in this position.


Required Skills:

Conduct patient evaluations histories and physical exams for new and follow-up cardiology patients Manage and monitor patients with cardiovascular conditions such as hypertension coronary artery disease atrial fibrillation heart failure and hyperlipidemia Order and interpret diagnostic tests including EKGs Holter monitors echocardiograms and stress tests Assist in care coordination for procedures or specialty referrals Provide inpatient cardiology consults and follow-ups (if applicable) Participate in hospital rounds discharge planning and transitions of care Educate patients and families on lifestyle changes medications and disease management Collaborate closely with supervising cardiologists primary care providers and support staff


Required Education:

Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in Hawaii or eligibility to obtain one National certification (AANP ANCC or NCCPA) BLS and ACLS certification required 12 years cardiology or internal medicine experience preferred; strong new graduates with relevant rotations or RN cardiology background considered Excellent interpersonal and communication skills Comfortable working independently with physician support EMR experience
