Job Description:
Assists licensed professional staff members in assessing the mental status of patients. Leads organizes and participates in therapeutic interactions guidance support and role modeling. Participates as a member of the care team and implements assigned treatment plan interventions. Communicates with patients in various situations small groups activity groups and as directed. Assists clinical staff during group activities. Monitors and reports any unusual behavior or incidents.
Requirements
Minimum Qualifications:
- High school diploma or General Education Development (GED).
- Minimum two years of past work experience in this position.
Required Skills:
Conduct patient evaluations histories and physical exams for new and follow-up cardiology patients Manage and monitor patients with cardiovascular conditions such as hypertension coronary artery disease atrial fibrillation heart failure and hyperlipidemia Order and interpret diagnostic tests including EKGs Holter monitors echocardiograms and stress tests Assist in care coordination for procedures or specialty referrals Provide inpatient cardiology consults and follow-ups (if applicable) Participate in hospital rounds discharge planning and transitions of care Educate patients and families on lifestyle changes medications and disease management Collaborate closely with supervising cardiologists primary care providers and support staff
Required Education:
Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in Hawaii or eligibility to obtain one National certification (AANP ANCC or NCCPA) BLS and ACLS certification required 12 years cardiology or internal medicine experience preferred; strong new graduates with relevant rotations or RN cardiology background considered Excellent interpersonal and communication skills Comfortable working independently with physician support EMR experience
