Kyabram District Health Service (KDHS) is a medium-sized rural health service that delivers a range of programs and services including acute subacute residential aged care and allied and primary health care services. KDHS has three campuses located in the townships of Kyabram Stanhope and Tongala.

the Role

Kyabram District Health Service is seeking an experienced and motivated Social Worker to join our Primary Health team. This dynamic role delivers integrated social work and counselling services across community aged care and acute settings supporting individuals and families at all stages of life.

You will provide comprehensive assessment counselling crisis intervention and advocacy working within a person-centred framework that recognises the social determinants of health. The role involves managing a diverse caseload and delivering evidence-based interventions that are responsive compassionate and outcome-focused.

Working collaboratively within multidisciplinary teams you will actively engage with internal departments and external service providers across the Campaspe Shire to strengthen referral pathways and enhance coordinated care. You will advocate for clients link them with appropriate services and contribute to health education and promotion initiatives.

This position also offers the opportunity to provide peer support and supervision to colleagues and students and to contribute to policy development quality improvement and best-practice service delivery.

About You

You are a qualified and experienced Social Worker with a Bachelor of Social Work and eligibility for membership with the Australian Association of Social Workers. You bring demonstrated experience in assessment crisis intervention counselling and service planning with the ability to manage a diverse caseload across varied clinical settings.

Your practice is grounded in person-centred principles and informed by social models of health. You understand the impact of social determinants on wellbeing and are skilled in delivering evidence-based therapeutic interventions that support positive and sustainable outcomes. You are confident working with individuals families and carers including those experiencing complex psychosocial challenges.

You have a strong understanding of ethical practice professional standards and relevant legislative requirements and you demonstrate sound clinical judgement in sensitive and high-pressure situations. Your communication skills are highly developed enabling you to build trusted relationships with clients colleagues and external service providers.

You are collaborative adaptable and committed to continuous improvement. Experience in regional or rural practice mental health therapies or working with children and young people will be highly regarded.
