Job Summary
Eden HHC is looking for experienced Licensed Vocational Nurses to join our dedicated team. The candidates must have minimum of 1 year professional Home Health nursing experience. Our service area includes Los Angeles County Orange County Ventura County San Bernardino County Riverside County and Kern County. We use Wellsky/Kinnser for EMR. This position is for Per Diem visiting nurse; Candidate will set preferred areas to cover.
Candidates must have:
- Current active LVN license in the state of California
- Current CPR Health clearance TB clearance
- Minimum of one year of professional Home Health nursing experience
- Must pass criminal background check
- Drivers license
- Proficiency in EMR charting and timely submission of documents
- Proficiency in wound care catheter insertion tracheostomy care diabetic care vital signs monitoring ostomy care
Responsibilities:
- Complete on-going comprehensive assessments of the patients needs
- Provide service and /or treatments requiring substantial and specialized nursing skills
- Provide skilled nursing care under the direction of the RN or the physician
- Educate and support the patient family and caregiver in meeting nursing and related needs
- Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.
- Implements the patients individualized plan of care and participates in case conferences
We offer a very competitive pay: $40 per visit
