Heres What You Need
Masters degree in social work Counseling Rehabilitation or Education Or
bachelors degree in nursing Or
Bachelors degree in business or Related field Or
Bachelors degree in health/science or related field Required.
Two (2) Years leadership experience (formal or informal) Required.
Two (2) Years Practicing Case Manager experience preferred.
One (1) year of brain injury rehab preferred.
LMSW - Licensed Master Social Worker Or
LPC - Licensed Professional Counselor Or
RN - Registered Nurse Or
PT - Physical Therapist Or
OT - Occupational Therapist Or
SLP - Speech Language Pathology Upon Hire Preferred.
BCLS - Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly Required.
Knowledge of regulatory and government agencies including The Joint Commission CARF Texas Department of Insurance DWC and DARS.
Knowledge of continuous improvement and change management. Good verbal and written communication skills in English; Problem solving ability.
Ability to teach and coach others. Time management and organizational skills.
Good interpersonal and customer service skills Leadership skills; flexibility; ability to independently manage complex case situations.
Knowledgeable about community resources state agencies and services; knowledgeable about payor issues.
What You Will Do
Oversee the Outpatient programs and processes working closely with the Managers and Coordinators aligned with the OT PT and SLP staff.
Oversee the Case Managers/Care Transition Managers working with outpatient cases.
Responsible for the Outpatient patient experience conflict resolution and handling any complaints.
Liaison between the outpatient and scheduling department.
Performs age-appropriate patient assessment and establishes plan of care.
Performs age-appropriate patient treatment.
Demonstrates Professional Accountability.
Performs Appropriate Documentation.
Coordinates day-to-day operations of all outpatient therapy disciplines.
Assists department leadership in the ongoing assessment of customer needs the assessment and modification of current programs and the development of new programs.
