Description
Job Summary:
Contributes to excellence in patient/family centered care by coordinating the health care of select patient populations/designated services across the continuum of care using the nursing process. Promotes the safety and well being of all patients by coordinating the patients discharge from the hospital and transition of care to the out-patient clinic. Collaborates with the inpatient interdisciplinary team to develop and maintain the plan of care.
Primary duties and responsibilities:
Discharge and Transition Management:
1. Promotes the safety and well being of all patients by coordinating the patients discharge from the hospital and transition of care to the out-patient clinic.
2. Collaborates with the inpatient interdisciplinary team to develop and maintain the plan of care.
3. Works in collaboration with others to ensure that all discharge needs are met.
4. Schedules all discharge appointments treatments and protocol specific orders.
5. Completes needed discharge follow up phone calls to patients and documents appropriately.
Professional Development:
1. Works with manager to formulate plan for professional development.
2. Attends educational in-services as appropriate.
3. Participates in professional activities and organizations to maintain knowledge of current trends practices and developments.
Performs other duties as required.
Qualifications:
Minimum required:
Education - Bachelors degree in Nursing.
Experience - Three years of clinical nursing experience as a Registered Nurse with at least two years of recent experience (within the last five years) in the area of concentration.
Licensure - Must have a valid active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.
Certification -
1. BLS
2. If completing virtual care activities that may include multi-state practice an active compact/multistate license (eNLC) is required within 60 days of hire. Employees in role prior to 11/09/2025 will have until their next renewal date to achieve compact status.
Preferred Qualifications:
Knowledge skills and ability requirements (preferred):
1. OCN certification preferred for some positions.
2. ACLS Certification preferred but may be required in some areas.
Physical Requirements: (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs 34-66% of the workday (frequently); 01-10 lbs 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking Occasional sitting Close eye work (computers typing reading writing) Physical demands may vary depending on assigned work area and work tasks.
Environmental Factors: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock Floor Surfaces Hot/Cold Temperatures Indoor/Outdoor conditions Latex Lighting Patient care/handling injuries Radiation Shift work Travel may be required. Use of personal protective equipment including respirators environmental conditions may vary depending on assigned work area and work tasks.
