Sentara Home Care is seeking to hire a qualified individual to join our team as a Care Transition Coordinator.
Position Status: Full Time - Day Shift
Position Location: This is a float position requiring coverage for the day at an assigned hospital. Mileage reimbursement is offered for travel to select hospitals. Hospitals are located throughout Chesapeake Virginia Beach Norfolk Williamsburg Newport News and Hampton.
Hourly Range: $28.16/hr - $46/hr
We provide market-competitive compensation packages inclusive of base pay incentives and benefits.
Overview
Coordinates and directs the referral activities from health care facilities to home care by identifying/determining appropriateness of care gathering medical and financial data meeting with patients and families and working with care management physicians and referral sources to ensure a smooth transition. Responsible for safe and effective transfer from facility to home and securing patient referrals by maintaining customer relationships.

High School Diploma (Required)
Certification/Licensure
Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI
Valid license in state of practice :

Experience
1 year of home care case management or discharge planning experience (Required).
