Short Description
- Facilitates coordination for reconstructive and general plastic surgery needs collaborating with plastic surgeons and surgical teams to ensure appropriate referrals pre-operative preparation and continuity of care for patients requiring complex wound closure soft tissue reconstruction or post-oncologic reconstructive procedures.
- Coordinates the continuum of care for breast cancer patients undergoing surgical treatment facilitating seamless collaboration between breast surgeons and plastic surgeons to support timely reconstruction planning and optimal clinical outcomes.
- Serves as the central point of contact for patients guiding them through surgical decision-making reconstruction options and treatment timelines while ensuring clear communication among the multidisciplinary care team.
- Streamlines pre-operative and post-operative coordination including scheduling education and preparation for procedures to improve patient experience and reduce delays in care.
- Provides patient education and navigation support helping patients understand surgical pathways choices and recovery expectations while addressing barriers to care.
Experience Required
Minimum of 3 years surgery nursing and/or care coordination experience and background a plus.
Education Requirements
License/Certification Requirements
Current NJ-RN License (Registered Nurse) BLS and ACLS required.
