Evaluate and complete assessments and/or consultations on high risk patients including those patients on enteral and parenteral nutrition and/or those identified as needing services on a timely basis per department/hospital policy. Potential drug-food interactions electrolyte/labs values diet order anthropometrics appetite physical assessment nutrition history medications and medical history are reviewed analyzed and summarized. Assessment includes accurate calculation of calorie protein and fluid needs based on pt s age sex and medical condition level of activity MNT goals and lab interpretation. Identify patient nutritional status and nutrition related problems; develop care plans and nutrition goals. Coordinates communicates and documents with appropriate interdisciplinary team member verbally and in medical record. 50% 2. Educates in-patient and out-patients on therapeutic diets based on department guidelines. Includes learning and age-specific needs drug-food interactions life-style ethnic/religious and economic considerations. Evaluates effectiveness of counseling based on objective information. The individual s progress toward achieving nutrition goals should be detailed in the medical record and communicated to appropriate health care professionals. Documents education provided in medical record. Provides for follow-up care as needed as well as communicating with outside agencies as appropriate. Revises and completes nutrition education materials as necessary. 25% 3. Provides services to the ESRD (end stage renal disease) patients receiving outpatient dialysis services. Services include initial evaluation and assessment short and long term care planning monthly follow-up and care conference. Communicates with outside agencies regarding patient needs as appropriate. Services are timely accurate and complete. 5% 4. Calculate and develop therapeutic diets and menus based on the approved diet manual. Evaluates the diet prescription relative to diagnosis and recommends changes as appropriate. Serves as a resource for Food & Nutrition Services staff and registered dietetic technicians. 5% 5. Serves as a resource and preceptor/mentor for medical students dietetic students and interns allied health trainees and hospital staff. Develops and presents lectures in-service training poster sessions and/or other learning activities as appropriate to students and hospital staff. Participates in hospital committees as assigned. 5% 6. Completes other duties as assigned. 5% 7. Orders or changes Medical Nutrition Therapy orders such as a change in diet order oral nutritional supplement or enteral nutrition product/rate/volume in the EMR per policy and within the Dietitians scope of practice. 5% Qualifications/Knowledge Skills & Abilities Education/experience/licensing: Bachelor s degree in dietetics or food and nutrition with completion of dietetic internship. Master s degree and membership in Academy of Nutrition and Dietetics preferred. Completion and maintenance of registration as a dietitian as provided for by the Commission on Dietetic Registration through the Academy of Nutrition and Dietetics. Will accept eligibility to complete Registration provided it is completed within 3 months of hire. Licensed in state of Ohio to practice dietetics as provided by the Ohio Board of Dietetics. One year work experience in hospital setting as RD LD preferred. Specialty certification in nutrition support renal oncology gerontology or diabetes education desirable. Demonstrates competency in the nutritional care of patients including age specific learning and cultural/religious needs. Adheres to the standards of professional practice as defined by the Academy of Nutrition and Dietetics
