Job Summary
Posted for a transferring current traveler. Title: Care Coordinator (case manager) nurse positions to collaborate with Nursing and Providers to support patient care within our NahataDziil Clinic in Sanders AZ. Shift: Weekdays Monday through Friday from 8a to 5p (no weekends or holidays). Housing: There is limited housing available in Sanders AZ but the nears cities would be Gallup NM or Holbrook AZ (Hotels and RV Parks). Information can be found under the Facility Group Document section. Orientation: Every other Mondays only - If your traveler misses the compliance deadline they will pushed to the next scheduled NEO every other Monday
Shift: M-F 8-5 no holidays or weekends
