Job Summary (Physical Therapist Assistant):
- Provide physical therapy services under the supervision of a licensed Physical Therapist (PT) in a skilled nursing facility (SNF) setting.
- Implement treatment plans to enhance residents mobility strength balance and functional abilities.
- Deliver direct therapy interventions including exercises gait training and balance activities.
- Monitor and report residents responses and progress to the supervising PT.
- Assist with resident assessments and contribute to care planning and documentation.
- Educate residents and caregivers on exercises and safety techniques for use outside therapy sessions.
- Maintain accurate and compliant documentation in the electronic medical record system.
- Ensure proper use and maintenance of therapy equipment and assistive devices.
- Collaborate with interdisciplinary staff during team meetings to coordinate resident care.
- Adhere to all regulatory and professional standards and maintain licensure and continuing education.
- Work as part of a rehabilitation team in a physically active fast-paced environment involving direct resident care.
