Job Summary
Position: RN Oncology Nurse Navigator Facility: Pagosa Springs Medical Center Location: Pagosa Springs Colorado Department: Oncology / Infusion Services Start Date: ASAP Contract Length: 13 weeks Shift: 4x10s (40 hours per week) Monday - Thursday 7:30 a.m. - 5:30 p.m. No weekends or holidays Guaranteed Hours: 40 hours On Call: None anticipated Weekends: None anticipated Floating: Not required EMR: Cerner Position Summary Pagosa Springs Medical Center is seeking an experienced Travel Oncology Nurse Navigator to support our Oncology and Infusion Services team. This role combines oncology patient navigation with direct outpatient oncology nursing support. The Nurse Navigator serves as a central point of coordination for oncology patients throughout diagnosis treatment and follow-up. The position also supports infusion and clinic workflows ensuring patients receive timely care education and care coordination across the continuum. Responsibilities Coordinate care for oncology patients across the treatment continuum Provide patient education regarding diagnosis treatment plans and symptom management Collaborate with oncology providers and interdisciplinary care teams Assist with infusion clinic operations as needed Perform IV starts and support port/PICC line access when required Support chemotherapy/biotherapy treatment workflows Coordinate imaging specialty referrals and prior authorizations Assist with oral chemotherapy medication coordination and patient monitoring Provide psychosocial support and connect patients with community resources Maintain accurate clinical documentation and care coordination notes Minimum Requirements Active Registered Nurse license Minimum 2 years of Oncology Nurse Navigator experience Strong oncology nursing background ONC certification required Experience working with oncology patients in an outpatient or infusion setting Strong patient education and care coordination skills Preferred Qualifications ONS Chemotherapy/Biotherapy certification Experience with infusion nursing Experience with port or PICC access Prior oncology navigation program experience Facility Description Pagosa Springs Medical Center is a rural community hospital serving the Four Corners region. Our oncology program focuses on delivering high-quality patient-centered cancer care while supporting patients through every step of their treatment journey.
Shift: Monday-Thursday: 7:00 a.m. to 5:30 p.m. No weekends or holidays
