Position Overview:
Provides nursing care to patients under the direct and indirect supervision of the Senior Nurse Manager or designee. Provides care for patients in common clinical situations utilizing advanced nursing practice knowledge and skills. The position requires good interpersonal skills and the ability to communicate effectively at all levels. The LPN must be able to perform the essential functions of the job with or without reasonable accommodation.
Key Responsibilities
- Perform comprehensive patient assessments collect data and complete required forms according to center standards identifying overt problems needs and priorities.
- Observe and record latest diagnostic results and perform advanced nursing observations using critical thinking skills.
- Make appropriate referrals to multidisciplinary support services and follow the patient care plan developed by the RN.
- Provide correct treatments procedures and care as prescribed and in accordance with patient care standards and accepted practice.
- Demonstrate understanding of age-related characteristics and needs of patients served.
- Explain nursing procedures appropriately and provide discharge teaching to patients.
- Identify problems requiring emergency measures and demonstrate respect for patient rights utilizing established mechanisms for managing ethical issues.
- Record patient care delivered as planned including any variations and rationale and make observations related to impending or associated problems.
- Delegate nursing care to appropriate personnel using the nursing process and integrate cost-effective measures into nursing practice.
- Recognize unit problems document them appropriately and participate actively in Quality Improvement (QI) processes.
- Comply with center expectations to meet staffing demands based on patient care needs and adhere to policies addressing safe working conditions.
- Monitor for unsafe conditions recognize inappropriate patient care management resolve issues and complete written reports as necessary.
- Perform all other duties as assigned.
Qualifications & Requirements
- Licensure: Must be a graduate of an accredited school of practical nursing and possess a current Louisiana license to practice nursing.
- Experience: Prefer applicant have at least 18 months LPN experience with 12 months in geriatric medicine. Must have practiced nursing in the last 3 years.
- Skills: Strong clinical assessment and critical thinking skills; ability to work effectively within a team; excellent verbal and written communication abilities; must have successfully completed an approved I.V. therapy course.
- Compliance: Must adhere to all applicable state and federal regulations and organizational standards.
