The UW Medical Center-Montlake Kidney and Liver Transplant Departmenthas an outstanding opportunity for a Social Work Assistant.

WORK SCHEDULE

Full-Time / 40 hours per week
Day Shifts

POSITION HIGHLIGHTS

The Social Work Assistant serves as a member of the Kidney and Liver Transplant Social Work team primarily assisting with the coordination of needed resources for patients
This team is a dynamic socially complex fast-paced service
Responsibilities include working closely with Social Workers patients and their families and the medical team in helping them follow-through on community resource referrals tangible needs and application processes for necessary service
Ideal candidates have experience working in a medical setting demonstrate strong multi-tasking and organizational skills and work well with a variety of persons and teams
Previous Social Work Assistant experience is preferred

PRIMARY JOB RESPONSIBILITIES

Facilitate the placement of patients in institutional settings based on data obtained through clinical staff.
Arrange referrals to home health based on referral from Social Worker.
Arrange and coordinates the transfer of patients including arranging transportation.
Assist the Social Worker and the patient/family with locating housing.
Assist the patient/family with emergency needs for resources such as emergency funds food vouchers and transportation subsidies.
Provide general clerical assistance with faxing copying and coordinating paperwork to facilitate discharge.
Research and update community resource information as needed.
Assist patient/family with financial applications in coordination with UWMC financial counseling and DSHS Case Managers.
When appropriate assist the Social Work team in screening for social work involvement.
Maintain communication with UWMC health care team on patient status/needs via the medical record and daily consultation.
Prioritize work activities based on needs of the Social Work teams.
Provide statistical reports to Social Work administration on SWA referrals and activity.
Obtain insurance authorization for services as requested by Social Worker.

REQUIRED POSITION QUALIFICATIONS

Bachelor of Social Work or a Bachelors Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent.

an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.

Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements such as a license/certification/registration.
